services/horizon: Work with Jesse to create a survey form and set of questions for our Horizon partners

What problem does your feature solve?

Since we are currently focused on improving the documentation and partner experience in general, it would be a good idea to do a survey/set of 1-1 meetings with our Horizon partners to understand their setup, how they are deploying Horizon and what issues they are facing. We can also ask them what parts of the documentation are hard to understand and improve accordingly.

We have done some similar surveys in the past:

but:

1. These are old and do not give an updated look at the partners' infra and use-cases.
2. They are not consolidated in a single place which we can use to refer to in case of incidents (e.g. the recent Horizon ingestion issue: https://docs.google.com/document/d/1rzuvcXFhXHVf_L_1UBxmCbADD7reEKxNhPnZOj2_ZDg/edit#heading=h.b5xdh739mlq)
3. Very little information exists about the Horizon infrastructure setup of these partners. We have done a survey of endpoints but not a lot of focus on the infra setup.
