Maybe this can be done with a new relay message type. Client can encrypt the image just once and upload it to meme-server, and store the random key. Then anytime they want to connect, encrypt that key and send it to the new contact so they can download the image.

What about the UX though? Should people still be able to choose their own profile pic for their contacts if they want to?
