Only amend CSP policy with Discourse server URL if user came from there and on a relevant route #2

Following up to #1, can the configured Discourse server URL only be output on routes that actually need it, for both minimizing attack surface and privacy?

Requirements

  1. Only output the header if the user has come from the Discourse server domain, i.e. has been redirected to log in, and
  2. Only output the header on log in routes:
    • user.login
    • tfa.login
    • tfa.entry?
    • user.register?
