I've been seeing this happen now and then but didn't pay a lot of attention to it. If adjusters are rezzed and the parent build is taken back to inventory or deleted, the Adjusters do not automatically die.
I've tracked that issue down and found that in the adjuster script, somehow we've not grabbed the parent key at all. In the timer the script checks if the parent is still around and if not, the adjuster dies automatically. Without the parent key, this does not happen.
