Pan tool stops working with wheel zoom

I don't have a good recipe for this, but it keeps happening so I'll just report it and if more information is necessary I can add it.

1. load annotator
2. activate wheel zoom and pan
3. pan works fine
4. zoom in/out with the wheel zoom
5. pan no longer works
6. deactivate all tools and reactivate
7. pan works again

It only seems to happen upon the first draw of the annotator (i.e. when deactivate and reactivate the pan tool it stops happening). If that recipe doesn't work for you, I won't be surprised. Its never really consistent so its hard to narrow down, but my best guess is that its some interaction between the pan and wheel zoom.

I suspect you are right and there is some issues with tool activation in merged toolbars.
