* refactor: rename open-amt-cloud-toolkit to device-management-toolkit

* refactor: update URLs to new organization name (#378)

- Org changed from open-amt-cloud-toolkit to device-management-toolkit
    - Fix markdown lint errors and use link references to repeat URLs
