After we retire boost-1.58.0 we should cleanup the hacks around expected failures #93

The mechanism to specify that a unit test has expected failures (for example a test for a testing function) changed between 1.58.0 and 1.59.0. We have a supported platforms (ubuntu 16.04) which uses boost-1.58.0 by default.
When boost-1.58.0 is retired, we need to cleanup some code.

See #93 for details.  Until we retire support for boost-1.58.0
we need to use two different styles to indicate that a test
has expected failures.
