Reception Fixes

• Fixes patient middle being null in Test Details and Order details sections of recepetion and main IBLIS tests list
• Removes readonly from login inputs on Reception allowing keyboard entries
• Adds view port optimization on tests types multi-select to allow scrolling to bottom
