As of now, all NPCs take up one tile of space. I know that there was some discussion before about the possibility of changing that, but nothing ever came from it. I'm confident in saying that every single game made in this engine will eventually want an enemy larger than 1 tile, and it feels very janky and weird having a large enemy who's hitbox is only his feet.
I'm not sure of the easiest way to go about implementing it, but off the top of my head a simple tile selector similar to the Projectile Editor (minus the directional arrows) would work well in the NPC Editor.
