[GENERAL]: Changes in layout of booking form

Problem:
The booking form layout needs adjustment to improve alignment and spacing. Currently, the fields are not aligned properly. It will look better in the form of a kind of form like having questions in vertical format instead of a horizontal grid. I would love to make changes if you assign the task to me.
